The only source document info I can find on HUD Elderly Housing inspection records on Linden House are for 2001, 2004 and 2007. 

An inspection on 8/14/2007 has a dwelling unit score of 71.17, with an overall inspection score of 29.32. 

An inspection on 10/26/2004 has a dwelling unit score of 87.06 with an overall inspection score of 61.7

An inspection on 7/17/2001 has a dwelling unit score of 96.29 with an overall inspection score of 92.54 

One Affordable Housing search website states that " Since the property received a failing score of 29.32, it has been referred to a HUD Enforcement Center for corrective action."

Good luck finding what you're looking for on the HUD Enforcement Center website. I couldn't.

This from a friend of mine who is regulatory compliance officer for numerous senior properties with which her company has contracts with HUD. 

There are two types of inspections that HUD does:
* Management and Occupancy Review (MOR) - this one is primarily checking if the property is accurately completing its compliance responsibilities and that the property is well managed. MORs haven't been done on a lot of buildings in the past 3 or so years because HUD has not got contracts in place with Contract Administrators (CAs) to do the inspections. 
* REAC - this is the very thorough physical inspection. If a property gets a high enough score (in the 90s, I think), it will only have a REAC every 3 years. A bit lower score (in the 80s?) will mean every 2 years, and below that every year.
We have one or two buildings that haven't been REAC'd in quite a few years, not sure why. (emphasis mine)

What does the * signify in the REAC scores? There doesn't seem to be an explanation on the page.

Sorry. The code on the numerical results indicates that there were both "At least one inoperable smoke detector noted" and "At least one Life Threatening health and safety deficiency noted" at Linden House. 

Inspection codes

a- No health and safety deficiencies noted

b- Non-Life Threatening health and safety deficiencies noted

c- At least one Life Threatening health and safety deficiency noted

*  - At least one inoperable smoke detector noted
